Stroke and Neuro Rehabilitation Follow-Up ProgramWhat is the Stroke and Neuro Rehabilitation Follow-Up Program?The Stroke and Neuro Rehabilitation Follow-Up Program provides an appointment with a doctor who specializes in recovery of function after a stroke. This doctor is called a physiatrist. The appointment would occur 2-4 weeks following your hospitalization or subacute stay. The physiatrist will evaluate you to determine if there are any new or worsening problems that could benefit from treatment to improve function and quality of life after your stroke. If needed, a physical therapist, occupational therapist and speech therapist will be available for evaluation. Why should I attend the Stroke and Neuro Rehabilitation Follow-Up Program?Many people who have had a stroke notice functional problems that might begin several weeks or months after they are discharged to home. The Stroke and Neuro Rehabilitation Follow-Up Program creates a system to treat these problems. You decide, together with the physiatrist, when and how often you need to return to prevent complications. How to I register for the program?A coordinator will call or visit with you, explain the program and schedule an initial appointment. About a week before the appointment, a reminder call will occur. You will be asked some questions about how things are going at home to help us determine what the physiatrist should focus on during your appointment.
